Authorization
To authorize the release of medical information to an individual or organization, please submit the Authorization to Disclose Health Information to the Campus Health Center.
Communication
Due to legal and ethical guidelines, as well as confidentiality, we are unable to provide medical and/or diagnostic services to anyone via email or other electronic communication device.
Please note that email is NOT a secure form of communication. Therefore, we urge you NOT to email any medical documents. Medical records should be sent via United States Postal Service directly to the Campus Health Center at CSUMB, Health & Wellness Services, 100 Campus Center, Seaside, CA 93955-8001.
