Please note that fees for service are due and payable at time of service.
The Campus Health Center accepts cash, check, money order, Visa, Master Card, Discover, American Express, and ATM/Debit cards.
We can bill some PPO medical insurance carriers or provide you with a statement that you may submit to your carrier for reimbursement. Unfortunately, we are unable to bill HMOs.
Please note that fees for services are due and payable at the time of service. In order to receive discounted rates you must have a current student ID.
- No charge for "basic" level medical services and some diagnostic lab tests performed at the CHC and as defined in Executive Order #943 (office visits, exams, in-house lab tests).
- Other services are billed at a discounted rate, such as supplies, medications, lab tests, immunizations, and allergy injections. (Note: X-rays are available at discounted rates through an off-site Doctors on Duty Clinic). Students are responsible for the remainder of DOD charges.
- No charge for the first copy of a student's medical records. Please allow a minimum of 24 hours for records on-site and seven to ten working days for records archived off-site.
- Continually missed appointments may result in the student being unable to schedule future appointments and only able to utilize daily clinic walk-in hours.
- Charges for birth control pills and other medications vary. Please allow at least 48 hours from the time you call before your prescription is filled. Please pick up in-house prescriptions at least 30 minutes prior to the Campus Health Center closing.
- The Campus Health Center pharmacy is only able to fill prescriptions that were originally prescribed at the CHC. It is not a retail pharmacy and does not sell over-the-counter medications or devices.

*NOTE: Whenever possible, students should have lab tests performed at the Campus Health Center as some tests processed by Quest Diagnostics are discounted ONLY when collected at the Campus Health Center. Otherwise, lab tests collected at a DOD clinic are billed to the student at the lab's standard, non-discounted rate.
CSUMB students with a current student identification card qualify for discounted pricing. Students must present a current student ID card for Fall, Winter, Spring, or Summer.
Dependents and CSUMB staff and faculty do not qualify for discounted pricing as they do not have a student identification card and do not pay the student health fee.
