Dental coverage effective date & Cost
The PPO plan offers coverage in and out of the network, so employees won't have to change their dentist. "In-network" coverage is provided for Delta PPO providers. If the dentist isn't a PPO dentist, coverage would be provided without network-level coverage (percentages); however, there is still a great benefit because most dentists out of network are participating in the Delta Premier network, 82% on average in CA.
If the dentist is contracted with Delta (PPO or Premier), they can process claims directly and cannot balance bill employees as they have contracted fixed fees with Delta. Delta Dental pays based on the date included on the claim form.

Vision
The Corporation pays employees and all eligible benefits 100% of the premiums. Enrollment is not automatic. The plan provides the following benefits:
- Eye examination - once every 12 months ($10 Copay)
- Standard lenses*/or contact lenses - once every 12 months
- Standard frames every 24 months - Coverage is available every 12 months if there is a change in prescription
Employees may use a participating or non-participating provider. However, if you use a non-participating provider, your benefits will be paid at a reduced rate. Refer to the plan booklet for copay and maximum allowance details.
You don't need an ID card or a claim form. Just contact your VSP doctor and tell them you are a VSP member.
Vision coverage effective date
The effective date is generally the first of the month following the employee's start date.

The retirement plan offered by the University Corporation at CSU Monterey Bay is a defined contribution plan (TIAA/CREF). The Corporation provides a 10% employer contribution, with a six-month waiting period and immediate vesting upon eligibility.

Basic term life insurance and accidental death and dismemberment insurance
Each eligible employee is covered for $50,000 at no cost, with an additional benefit for accidental death and dismemberment.
Optional life
Optional life is a voluntary program that allows you to purchase life insurance in addition to the basic life provided by the Corporation ($50,000) for yourself, your spouse, and your dependents.
Long-term disability insurance
This plan provides partial income replacement benefits for disability resulting from injury or illness sustained on or off the job following 180 days of disability. LTD benefits are coordinated with other benefits plan payments up to age 65. Coverage is automatic and provided at no cost to eligible employees.
Unemployment insurance
Unemployment insurance provides a weekly income to employees out of work through no fault of their own. If you become unemployed, contact the nearest Employment Development Department for eligibility and claim to file information.
Flexible spending accounts
University Corporation offers a Section 125 flexible spending plan. With this program, you may pay for certain medical and dependent care expenses with pre-tax dollars that have been set aside from your paycheck. You may participate in either or both medical and dependent care expense programs, but money cannot be "transferred" from one to the other.
Supplemental Insurance
American Family Life Assurance Company of Columbus (AFLAC) - is voluntary supplemental insurance providing cash benefits paid directly to you for medical bills incurred due to illness or disability not covered by health insurance. There are several plan options with pre-tax and after-tax premium deductions. An employee is responsible for 100% of the premiums.
Worker's compensation disability leave
University Corporation employees are insured for work-related illness or injury while on the job. Employees must report a work-related injury or disease to their immediate supervisor or University Corporation Human Resources within 24 hours of the damage. The University Corporation will grant a workers' compensation disability leave to employees with occupational illnesses or injuries by state law.
The employee must complete immediately following knowledge of injury or illness and submit to University Corporation HR.
This form, along with DWC1, must be completed, signed, and returned to University Corporation HR within 24 hr of injury.

Family and Medical leave
The Federal Family and Medical Leave Act and the California Family Rights Act provide employees with specific leave benefits. The Corporation intends that the policy described herein shall satisfy the provisions of both Acts concurrently. Regular employees with one year of employment are entitled to take a Family and Medical Leave. FML leave grants up to 12 weeks of paid or unpaid leave during a 12-month period to care for yourself, a child, spouse, parent, or domestic partner with a severe health condition. Medical certification is required. While on unpaid leave, the employee may request the Corporation to continue providing benefits coverage. The FML Act provides an employee with the right to reinstatement to the same or another position with equivalent benefits, pay, and conditions of employment upon the employee's return to work at the end of the FML. Other than for pregnancy disability, FML is concurrent with different types of leaves listed here. Refer to the Department of Labor's website for a fact sheet or a list of frequently asked family and medical leave questions.
Maternity/paternity/adoption leave
University Corporation employees shall be entitled to up to five (5) consecutive work days with pay for new parents for preparing for the arrival of a new infant and the care of a new infant. Paid maternity/paternity/adoption leave shall commence within sixty (60) days of the arrival of a new child. Such withdrawal shall be in addition to available sick and vacation leaves. Paid maternity/paternity/adoption leave runs concurrently with any other related leaves for which the employee is eligible under state and federal law.
Pregnancy disability leave
University Corporation employees who have become disabled due to pregnancy, childbirth, or related medical conditions, are eligible to take a Pregnancy Disability Leave (PDL) for up to four (4) months.
Leave without pay (LWOP)
An employee with one or more years of continuous service credit who works a regular schedule of 30 hours or more per week may request a personal leave of absence. The University Corporation does not pay your share of the foundation-sponsored insurance program while you are on personal vacation. However, coverage may be continued if you write to the Human Resources department 14 days before the beginning of the leave period to make arrangements for all premium payments.
Funeral leave
In the event of a death in your immediate family, you may take up to 5 days of paid time off from work to attend the funeral and to make any necessary arrangements.
Military leave
The University Corporation observes all federal and state laws regulating absence from work for military service. Therefore, an unpaid leave of absence is granted to any employee fulfilling reserve training, National Guard duties, or active duty service requirements.
Jury duty/witness
All employees on active employment status are eligible for jury duty. Time off with pay for jury duty will be only for those hours the employee was scheduled to work. You will receive your regular University Corporation paycheck while on approved jury duty.
Personal and sick time
The Corporation provides sick time to all eligible employees for periods of temporary absence due to illnesses, injuries, medical, vision, or dental appointments. Sick leave accrual may vary based on employee status and appointment.
Holidays
The Corporation normally observes thirteen full-day holidays per year, and each employee is entitled to one personal holiday. If a personal holiday is not taken by December 31, it is forfeited. Eligible employee classifications include: Full-time Regular and Part-time Regular benefited employees. Prior to the beginning of each calendar year, a list of the dates on which holidays will be observed by the Corporation will be distributed to all employees.
Vacation
Vacation is an accrued benefit that provides time off with pay with management approval. Eligible employee classifications include: Full-time Regular and Part-time Regular benefited employees. Vacation time may be used upon completion of one month of continuous service.
University Corporation at Monterey Bay offers preferred pricing on pet insurance Nationwide, providing coverage for accidents and illness. Plans are available for dogs, cats, birds, small mammals,s and exotics (such as reptiles); animals categorized as livestock (including horses) are not eligible.
You can enroll in pet insurance at any time — not just during Open Enrollment — and your coverage will be adequate approximately 14 days after nationwide approves your application. Premiums vary depending on your type of pet and where you live, and you’ll pay your premiums directly to Nationwide. Pre-existing conditions are excluded from coverage. All enrollment requests are subject to underwriting approval by Nationwide.

Suppose your pet is currently insured with Nationwide in a plan other than Ucorp. In that case, you will need to wait until your pet’s current policy is up for renewal to apply for enrollment in the Ucorp-sponsored group plan.

Employee assistance program (EAP)
The Corporation recognizes that life at work and home can be challenging. However, it's essential to know you are not alone. As a Corporation employee, you and any household member can turn to Hartford's Employee Assistance Program for help 24 hours a day, 365 days a year. In addition, you choose up to 3 in-person or phone sessions per incident with a provider from Hartford's extensive network to help you resolve a broad range of issues, including:

Otter Sports Center and Otter Aquatics Center
The Otter Sports Center houses a 10,000-square-foot gymnasium for basketball, volleyball, badminton, indoor soccer, aerobics, dance, sports games, and special events. There are also three racquetball courts, a 2,500-square-foot fitness room with cardiovascular and weight training equipment, and locker/shower/ sauna facilities. In addition, vending machines with snacks and beverages are on site. Please contact the Otter Sports Center at 831.582.3031 for the latest information.
The Otter Aquatics Center is an outdoor heated 25-meter by 25-yard eight-lane swimming pool that is NCAA short course swimming and diving compliant. The pool's depth ranges from 3 1/2 feet to 13 feet deep and includes two diving boards, a 1 meter and a 3 meter. Lap swim, open swim, and swim lessons are a few programs offered. Campus Health Center
The mission of the CSUMB Campus Health Center is to provide the highest standard of medical service while encouraging patients to be active participants in their health care. The Campus Health Center is primarily student-focused. The center's staff includes many professionals with expertise in primary care and women's health services. All clinical staff is licensed or registered in California.
The Campus Health Center is available to:
- All full-time or part-time CSUMB students who have registered and paid on-campus health fees
- CSUMB faculty and staff
- The dependents of CSUMB students, faculty, and staff
